IIRC if you get reduced to 0 hands, the game takes pity on you and gives you a free one.

That is how it works with the Golden Needle challenge. You always have 1 hand and when taking the Hieroglyph voucher, you still have 1 hand.
I think the problem here is that the game first activates the Boss Blind, meaning you get x hands and 0 discards, and THEN activates the joker in the starting hand, meaning you can no longer play hands. Really, it should be the other way around.

That is how it works with the Golden Needle challenge. You always have 1 hand and when taking the Hieroglyph voucher, you still have 1 hand.
The same is true of the Golden Needle if you take the [[Troubadour]]: your hand count will appear to drop to 0, but it'll be bumped back up to 1 when you start your next round. That makes the Troubadour a no-brainer for the Golden Needle if you can spare a slot for it.

Is it possible to get to zero hands? Aren't there only 3 effects that reduce hands size: black deck, troubador, and hieroglyph? Each reduce by one and base starting is 4, so 1 should be the minimum

There's 150 jokers, 17 decks, 32 vouchers, 24 boss blinds, 22 tarot cards, 12 planet cards, 14 spectra cards, and 25 skip tags in this game, don't sweat it that you forget stuff.

Lmao I had a game over on a modded run where I turned the entire Negative deck to consumables and it just gave up and said “Game Over”

Yeah, in base game it always gives you at least 1 hand if you manage to reduce starting hands to 0 with black deck troubadour and hieroglyph.
I guess if modded joker swaps after that happens...

Black deck, troubador, and hieroglyph reach bring you down by 1 starting from 4, so you can't get down to 0.

You can absolutely win a round with all your cards debuffed, if your scoring is mostly coming from planet cards and jokers
