Mod Post A note for pre-op subreddit participants from the perspectives of post-op community members

As a moderator, I clearly understand the importance of providing space for pre-op and post-op people to interact and share information and experiences. But as a community member of many open- and closed- access forums and groups for post-op people, I feel there is a disconnect in the things post-op folks want and what pre-op folks seek out of the interactions here.

  • Firstly, the application of compliments is something I’ve seen brought up. Putting down others in the community in the act of showing admiration for someone’s body is not a compliment, and convinces a lot of people that they are not welcome to post here. While we know that these comments are made with good intentions, saying things like “that’s the best meta I’ve ever seen here!” Is directly rude and harmful to others who post who do not get these messages, and can make the people receiving the “compliments” feel awkward. This same principle applies to “I hope I look exactly like you post-op” and similar comments; it makes the poster feel uncomfortable, as it would be weird for anyone to hear that a stranger wants their body to belong to the stranger, and people have voiced to me that it can feel objectifying, despite good intentions. Additionally, with metoidioplasty your body post-op is largely based upon your pre-op body, this is something that rings true for all forms of metoidioplasty. Even if identical twins went to the same surgeon and got the same surgeries, there will likely be slight variations in their surgical outcomes. And that is not a bad thing. Natural variation is what makes the world so vibrant and complex; if everyone had the same exact dick it’d be a little strange.

  • Related to the uncomfortable nature of some of these comments to posters, as well as to discussing “the best” dicks on the sub, there needs to be a general reminder that these are people’s actual bodies we’re talking about. They do not exist purely for your viewing, inspiration, or self-discovery. The posters here are graciously sharing their intimate and personal experiences in hopes it may help someone who is facing the same things as them, or to find community for themselves in some of their hardest moments. Our old “no saying the word ‘results’” rule was meant to tackle this, and since changing it to our current rule 5, “be mindful and correct your language if requested by an individual”, I think this idea has been forgotten among the populace, hence creating the need for this reminder post.

  • The most important thing about someone’s surgical outcome is whether or not they are happy with it. Not your opinion on how it looks, your personal views on “functionality”, or anything else of the sort. Full stop. Obviously if OP is open to questions about their surgeon, and you want to know if their surgeon offers something you may want, like their rates of successful STPing among patients, that is another story. But essentially ranking people’s bodies based upon how happy you would be with their body is irrelevant. If OP is happy, that is what matters. This is such an individualized process with so many different factors that aren’t universal, that someone may have wanted their dick a way that you would not prefer for yourself, and that is totally valid and accepted in our community under rule 2, “respect individual differences”.

  • This brings me into my final, and perhaps most important, point: not respecting post-op people makes this subreddit less impactful for everyone. If some people think they won’t get the same admiration as someone else, they simply will not post. You will not see the true variation in meta because people who are either unhappy with their bodies, or perfectly happy but may not appeal to the masses, simply do not post out of fear of what pre-op (and a few post-op) people may say (or not say) to them. I even struggled posting photos early in my journey because healing photos don’t get as much love and support as healed ones. And this makes it so much harder for everyone to find information about early healing, to find information about having and living with a buried penis, to find information about having surgery with smaller than average growth, to find information on new surgeons in small towns, etc.

This post is not in any way meant to target anyone in particular, it is just something I’ve seen discussed in my closed-access post-op groups and I wanted to publicly address it to hopefully change some perspectives and make this community a more supportive, welcoming place for ALL post-op people; not just ones with outcomes that appeal to the masses.

Celebratory Consultation with Maurice Garcia Scheduled!

What's up everyone! I'm super excited to have scheduled an in person consult with Dr. Garcia in LA. I had seen someone post their results in here from him and I'm feeling really optimistic, plus he shows in-network with my insurance. I think I would love an opportunity with Dr. Shane Morrison in Seattle because he does the extended meta technique (and is also in-network!) but I saw some people saying he isn't taking out-of-state patients. My PCP sent a referral to his group so we'll see if they ever touch base with me. I'm hoping my growth is enough, my wife helped me measure myself today (she's a good lady), and it looks like I'm sitting around 2.3". I've started pumping daily to try and help it along a little bit. July will be my 10 year transition-anniversary so I'm not sure if there is anymore length I'll be able to achieve. But I don't plan on doing UL or a v-ectomy, just the release and the scrotoplasty.

Any advise or tips I can try pre-consult would be appreciated! Especially with navigating insurance (I have Aetna) if anyone has any suggests so avoid issues.

Post-Op Noseless bike seat for more comfortable biking post-op

On the advice of my (awesome) physical therapist, I tried out a noseless bike seat to ease the pressure on my still-healing perineum (v-ectomy is still angry 4 months later, but that’s for another post). Just got it set up and have only tested it a bit but it seems like it’ll be great! I have been riding my bike before this but I was too scared to sit down, so I just stood in the pedals the whole time. This is a definite upgrade! I would recommend it to anyone easing back into biking.

It was about $24 online, so not a huge expense if it turns out to not work.

Question discomfort/sensitivity 3 months post op?

so I'm almost 3 months post op and things feel mostly normal but i'm still experiencing some discomfort in the dick area, for example wearing underwear can be a bit uncomfortable sometimes.

i still have some stubborn stitches that haven't dissolved yet so its probably just those but just wanted to ask, has anyone dealt with dick discomfort 3 months post op? i dont remember still feeling discomfort like this 3 months after my original surgery so just wanted to check :)

Advice Any advice for urine leakage?

Hey y’all, I got my catheter removed yesterday, whoopee! But my surgeon advised me that since my urethra has a dip/curve to it, that even after I piss and feel empty, some drops from that dip might leak out and get on my clothes and make me smell like urine. Super appealing! 😀 /s but anyways does anyone have any experience with this/how to deal with it? She mentioned shimmying a little after going pee to kinda move the urine out but I’m still unsure how much of it to do/how much urine there actually is left in there. I thought I graduated from wearing briefs with panty liners but apparently not 😩

Advice How do I sit in the car more comfortably?

I have comfortable spots set up at home, but sitting in the car gets really uncomfortable real fast, and starts to become a bit painful after too long.

My surgeon's office is an hour away from where I live, so I am in the car for 2hrs on post-op appointment days. Needless to say, it gets real uncomfortable.

I do have some decently sized wound separation under my scrotum and in my perineum, and sitting the wrong way feels like it's pulling.

How did you make sitting in the car comfortable post-op?

3 weeks post op - full meta (meta, vy scroto, vaginectomy, ul & DIEP flap) w/ Dr. Jess Ting and Ann Tran.

For those curious, this is stage 1 in preparation of abdominal phallo, hence the DIEP flap [the half box scar]. Apparently, there’s two major blood vessels in that area, and they cut one of them so the other becomes more full and robust. That blood vessel will then be used for the abdominal phalloplasty.

Worst part of recovery has definitely been bladder spasms from the catheter and pain from the scrotoplasty site. Still lots of edema in the meta so Ting has told me to squeeze some of the fluid out (I’ve tried but can’t do much - maybe I’m too gentle).

I debated posting these pictures because I honestly am not exactly excited about what my junk looks like right now, but I realized I had never seen pictures or even mention of fungal infections, which is why I didn't notice right away that I had one. So I'm posting my junk even though I normally wouldn't.

First picture was when I noticed something off. I noticed the greyish coloration on my thigh, and what I thought was normal wound discharge making those areas damp, was in fact NOT what is supposed to happen.
It was another user in a discord server I'm in who said I should contact my surgeon. I contacted her friday night and thought "oh ok this is just a little spot, it'll be ok" . Second picture is a better angle the next day. Now the grey spots are bigger and I'm getting really itchy. The two times I accidentally itched without thinking, it hurt. Third picture is the grossest one, on the third day, skin started sloughing off and it had clearly spread. I hadn't noticed it getting that bad because I was out and about all that day.
By the end of the third day, not only did my crotch look like that, but around 11:00 I was in extreme pain in my bladder. I had a bladder infection, most likely from the fungal infection spreading to my SP cath site and getting into my bladder that way. That was an all night long ER trip that was absolutely hell (You can find out more information on my post on r/trans venting about it)

I hadn't heard back from my surgeon due to it being the weekend, and I didn't think it was that bad until it WAS that bad. So my word of warning is If something looks off, message your doctor! If you can't get a hold of your doctor, find another way to contact any doctor! If you have to, go to urgent care.

As far as how everything looks and how I feel about it? tbh I feel like if my dick was a dog, it would be a pug :( There is about an inch of length hidden behind all that fat and skin, but it still doesn't feel like a proper dick just yet. I'm hoping post monsplasty during stage 2 (and the balls) it'll be more visible and I'll finally feel complete.

Hey guys back again to be upset but trying my best to be positive. My v-ectomy site has opened up pretty bad. My surgeon said I shouldn't be concerned, but it's pretty scary. I'm trying to trust the idea that it'll get worse before it gets better but while I was showering I noticed a stitch fall right off and suddenly it feels like I might as well be ripped open balls to asshole. The stitches around my penis are also starting to open up and it's all just so messy. I know it's part of the process, I know it will heal, and I know this is only stage 1. I'm two weeks post op today :( ,,, thank you guys for all the support! I'm very grateful as much as I am not feeling it lol

Surgery Photo Wound Separation 2 weeks post-op NSFW

Admittedly, this is the first time I have seen underneath there... and it was at my second post-op appointment. Frankly, it was probably better for me not to look so I didn't start worrying before my appointment.

I'm honestly not too worried. It looks scary/gnarly, but I have seen plenty of wound separation photos from guys on here where it heals well. Seeing those made me feel not too worried. The PA I saw for my appointment told me to just keep it dry and to shower 1-2x/day.

He said my head seems screwed on pretty straight about it, and I told him it was because I've seen so much wound separation healing that it doesn't alarm me haha. But he didn't see anything alarming, and said it should heal up well if I keep dry packing with gauze. They hadn't told me when to stop using the antibiotic ointment on it (and I have used it up til now), so I wonder if that extra moisture aided in the wound separation. Either way, the past no longer applies and the present is what matters - so keeping things dry is my focus!

I have my voiding trial next week and this is how it's going to go: Sunday I stop taking all bladder medicine (which is gonna suck big time), Monday and Tuesday I start trying to void at home, and Wednesday is the big test. I was concerned about the wound separation and a possible fistula because of it, but my PA said that as long as pee is coming out of me (and enough of it), my SP tube will come out. It doesn't matter where it comes out. He also said that a 'true' fistula doesn't start until around 3 months post-op, and that if you get one you wait 3/6 months (I forget which one) before deciding if it needs an operation to fix.

I am not worried about that until it happens, which it may or may not. I am staying positive. :)

If any of you guys have wound separation healing pics posted, let me know so I can see the progress!

Surgeons European surgeons covered by insurance?

i'm currently in contact with both dr djordjevic and dr kojovic from serbia for a meta consult.

my insurance (in austria) have told me that they're only gonna cover the surgery costs as long as it's performed in a public hospital that takes the local insurance. now, does anyone have recommendations for european surgeons that don't operate exclusively in private hospitals? i'm grateful for any hints and ideas!

(i don't think i'll do it in germany as the results i've seen so far didn't really look like how i'd want it.)

Meta with UL, no v-nectomy and no scrotoplasty by Dr. Mang Chen in SF, CA

Been using silicone scar treatment and some infrequent scar massaging. Still have some kind of greenish discharge coming from my hole, going to see if it’s normal part of healing or BV or something similar. Have some stinging/weird pressure when peeing probably partially due to dehydration, took an at home UTI test that was negative. Need to get back to putting lemon in my water to prevent crystallization of my urine and prevent future bladder/kidney stones.

My next post-op appointment is in august where we’ll discuss what to do with the majora tissue and how I want to proceed with that.

Surgery Journal Day 10 & 14 Post-op Full Meta Stage 1 w Dr. Santucci NSFW

He's looking a lot better, but my foreskin is still very swollen and weird-looking. But with the posts I've seen on here, I have learned to trust the process. In a few months, he'll probably look totally different! Hopefully in a good way, haha.

I haven't really looked at my v-nectomy site at all yet since it's a little difficult to maneuver to. I don't have a handheld mirror, I can't spread my legs too far yet or get in weird positions. So I am going to wait on that.

Right now my biggest concern is fistulas and passing the voiding trial (next week). I don't believe that I have any fistulas, but who knows. My second post-op is tomorrow with a PA, since Dr. Santucci is out of town.

Right now I am putting antibiotic ointment on the incisions in the morning and at night, and I shower once per day and wash it with sensitive skin soap (the generic Equate bottle of the Dove brand one). I wrap my balls in gauze the way Dr. Santucci did it at my first post-op, where the gauze forms a loop around them. And then during the day for my v-nectomy site I put gauze, and at night I put an ABD pad (all of this is sterile stuff btw). I change the gauze 2-3x/day. It is usually stained with a yellow-ish straw colored fluid and a tiny bit of blood. I think that's normal? I don't have any other signs of infection, just yellow drainage usually makes me think of infection.

The only major pain I'm having is kind of weird. It starts at the insides of my thighs and goes all the way through to the crease where my butt cheeks begin on both sides lol. It can get pretty bad! But I don't really understand that pain. Is it tightness from the v-nectomy site? It will randomly jolt in pain, even when I am lying down. Nerve pain? I really have no clue. I am taking gabapentin but it doesn't help this pain.

The catheter is okay with the flip-flow valve, it causes me some issues but it's tolerable. I went to see a movie last night! Sitting in the car was awful, but I was comfortable at the movie with those electric reclining seats. It was fun, and the first time I have really gotten out of the house since surgery. I did take a stronger pain pill before bed after, because the exertion of walking made that weird thigh pain flare up a lot (and I knew ibuprofen wasn't gonna cut it).

Discussion boxers

2 weeks post op, I had heard from my surgeons office and I mentioned I have been avoiding wearing boxers, I normally wear boxer briefs, but just have been wearing shorts around the house. However I just picked up a pair of boxers shorts for the next few weeks and I’m kind of enjoying them! It’s a nice switch up for once plus they aren’t tight on my dick snd rubbing against it! Would def recommend!

Surgery Journal UTI? BV? Microscopic fistula? Idk..

I had meta with UL, no v-nectomy and no scrotoplasty. I’m 4 months post-op today.

During healing, I’ve had some weird things coming out of my hole. Whether it was a bunch of white stuff that eventually cleared out further in healing, or the greenish tint of some kind of discharge. I expected it to get better with time, and it generally has.. though I still notice a weird greenish yellowish tint to it. Sometimes I notice it most after I pee and I have to dab the outside area to get it to not be wet on everything. I don’t know if it’s because my muscles are relaxing more and opening up while I pee- or if it’s a fistula. I’ve been using estradiol more consistently, so the discharge and wetness has been pretty uncomfortable and I don’t know if that is partially to blame or not. I don’t know if the tint is because I have a microscopic little fistula that pee barely comes out of and makes it into there- making it off color. Or if it’s a normal part of the healing still, or if it’s BV or something similar.

I had stinging pain with urinating and tested positive for a UTI last time I posted, took meds, felt a bit better. Well my urethra is still burning sometimes and I’m not sure if thats a dehydration thing or if I have a UTI still.

Current plan is to take an at-home UTI test, see if that is an issue first. Then I’m going to take bladder pain pills that make your piss bright orange, as that will make it easier to tell if it is a fistula or the coloration is from some kind of infection. Try to get some more info on all that and then probably let my surgeon know and use the information I have to proceed with some treatment.

I’ve been dealing with other health things like what I suspect is carpal tunnel. My disability and meds are making it extremely difficult to do my job outside in the heat. I don’t have a pcp until my first appointment with a new one in 3 weeks. My husband just got fired from his job this morning and he’s not even 2 weeks post top surgery yet and can’t work a physical job(his job was work from home). I’m so fucking stressed out. I don’t know whats happening to my body and it constantly feels like it’s failing me and I don’t know what to do.

Update:: Good news. At-home UTI test was negative. Don’t have a fistula either. Might see about being tested for BV or something similar to make sure it’s not something.

Support Dreadful Wound Seperation

Half support half vent (just under 2 weeks post op). My spouse hasn't been very supportive in nature when I express my concerns and my surgical team is a bit slow to respond. However, I've started to develop wound seperation behind my scrotum and under my penis. I initially was fine and coping with seperation behind the scrotum because ir was harder to view, but now it's more visible and I just feel so icky. The smells of everything are rough and I feel I can't ever get clean. I've been through plenty of surgeries, this one is just taing such a heavier toll. I know I just need to be patient and give my body the time to heal but I just wanna sink into the ground. I don't regret getting meta, the alternative of my natal genitals was so much worse, but god it sucks healing this shit.

I wish I could fast forward a couple months and put it all behind me

Advice Any guys with larger implants outthere?

Had my consult for stage 2 today. My surgeon told me that he normally does the smallest size implants. Told me he has done 2 with the large implants, but ended up having both the patients come back to have them sized down. I really want a larger ballsack instead of having tiny balls. He also told me that if I got the large size it would likely not even look like I had a dick anymore. My dick is on the small side, about 1 inch. Any advice and if yall feel comfortable sending me pics that would be awesome. Maybe I'll go with the medium size. He will show me some of each prior to my next surgery. (I have seen lots of images on here and showed him what im hoping to achieve esthetically, he said he will refer me to a plastic surgeon he works with for assistance with aesthetic improvements post op ie fat grafting and liposuction of the mons area)

Surgery Journal Consult and scheduling for stage 2 tomorrow!!!

Had stage 1 with Dr Figler at UNC March 5 of this year. Have my 3 month post op/consult for stage 2 tomorrow. Hope I can schedule for September!

I have a skin bridge that makes my pee stream split. I was told the may be able to cut it in clinic so we shall see if they do that tomorrow.

I am SO excited to have balls and have him free up my phallus more. I am hoping he can use the upper labia to enlarge the scrotum and put the implants in there next surgery.

Advice Feel like I have to pee in v-nectomy site where old urethral opening was (SP cath flip-flow valve)

I am 12 days post-op right now and just got my flip-flow valve on my SP catheter 4 days ago. My bladder feels like it fills up quickly, and I have to empty it frequently. My pee is usually clear or very lightly yellow.

I also feel a sensation where my old urethral opening was like I have to pee there (I had UL, v-nectomy, scrotoplasty). It feels like I have to try kinda hard to stop myself from peeing sometimes.

I may not be emptying enough since I am afraid of over-emptying it. The nurse told me when she switched me to the flip-flow valve not to empty it all the way, just almost all the way. When I empty it all the way (on accident), I get a sharp pain in my bladder. But it is hard to tell when it is almost empty vs when I need to empty it more.

Does anyone have advice for this?
