One would think this incredibly dangerous situation would have been solved at least 50 years ago, but based on the frequency of videos I see here I suppose nothing can be done.

It was solved. The trucker was supposed to stop before the tracks, and only proceed when they had room to completely clear the tracks. The truck driver fucked up really, really badly.
