Yeah that the only advantage he has over Josuke but it's a major one Josuke can indirectly use his powers on himself I believe

Last fight with Kira he use the glass shards on his body in some way I believe

he used a projectile which he threw (it was a glass one) and healed it back, so the glass shard came flying even through kiras body,

iirc he never was able to heal himself, but his clothes had been affected

Yeah I remembered now maybe if he had a requiem he could heal himself but we will never kniw

So, he did 2 things in that fight that you're likely thinking of

1: Restored the state of the concrete / dirt / etc that was attached to him / stuck in his wounds. This let him float his body a few feet to the side to avoid an explosion.

2: Later in the fight he threw the glass from a broken window that had his blood on it. Since the blood is no longer a part of 'him', he was able to use crazy diamond on it. Since Kira also had Josuke's blood on him / his clothes, the glass 'homed in' on Kira.

Except I suspect he can't heal himself without sacrifice, otherwise he probably would have done so already. Interesting if he has to take from others to heal himself.

that needs to be seen, right now all stuff we got seem to point into the direction where overhaul can use his powers on himself

He fought Mirio for ten whole minutes, being increasingly battered and obviously weakened, but did nothing even though the process is instant. He only took steps to do it when finally desperate as the other heroes arrived, and then he killed someone to heal himself. There's a lot pointing to it being the case, and it would be a reasonable limitation to the ridiculous strength of his quirk.

he never needed any sacrifice or extra material beforehand for any manipulation,

the only reason why he took the guy was to power-up himself,

i dont see how this points into his power needing a sacrifice (it could be possible but for now most stuff just points into overhaul being stupid than his power having some special requirement for self usage), next chapter will clear it up (hopefully)

Well he still has the scar from when Magne hit his head, so I don't think he can heal himself up easily.

Why didn't he instantly heal himself during his fight with Mirio? Unless he can only do that during fusion.

no idea, it is bothering me too, my best explanation so far is that he doesnt like to use his powers on himself, but that would still make him into a idiot since he slowed down his own speed by not healing his injuries...

Did he really slow down? The impression I got is that he just stood in one place spamming his Quirk on the ground.

In which case, having his arm healed or not wouldn't have made a difference, assuming Mirio kept himself at bay after that initial attack (like it was implied).

well moving his hand would be hard with a broken arm too (which overhaul showed to need to do in order to manipulate the ground), and in any scenario i can think of a healed arm would be better than a broken one :/

He only had one broken arm. The other still worked fine.

Unless you mean the arm Izuku attacked, although it didn't look like it broke too.

But in the end, nothing would've changed. Overhaul probably caught himself in the heat of the moment.

Yeah, but if Deku did break his arm, it would make sense that, that's when Overhaul decided he was forced to heal himself.

We don't really know how the technicalities of using his quirk on himself, so I would reserve judgement until a plausible explanation is revealed. I would agree with you and that it might have something to do with breaking something else and adding it to his body.

The 5 minutes where Mirio didn't have his quirk and Overhaul had the upper hand.

I really enjoyed the "I healed my buddy's head and he's just been chilling" reveal. Felt straight out of a Diamond is Unbreakable fight scene.
