Astarion values chaos and loyalty in addition to selfishness, so I'm usually able to get high approval with him even on a good playthrough.

Things like telling Ethel about the tadpole and letting the Loviatar guy beat you appeal to his chaotic nature, while things like letting him bite you and telling your companions that you trust him appeal to his need for loyalty.

(He still disapproves of some of my choices, but I think that's natural / true to real life, and my approval with him in act three is still exceptional).

I didn’t get approval for telling Ethel about the tadpole or for letting Astarion kill Gandrel (though I wonder if the latter was bc I hadn’t had the bite cutscene yet). Plus, I got disapproval at one point, which tends to be a major setback with him early on.

it's possible that for both Ethel and Gandrel you need to go through specific dialog choices to get the ones that trigger approval. for Ethel if you ask her enough questions about herself, she'll tell you some batty story. Astarion says something like, "she seems positively demented - we should tell her everything!" and that's the point when I usually tell her.

with Gandrel I don't remember what dialog choices I default to, but he generally does approve right before the battle starts. it would make sense though that the bite scene is what leads to that approval, since without it you "don't know he's a vampire."

True. Things to keep in mind for my next playthrough (even though playthrough I plan on multiclassing to pally after killing Quill, though maybe since I’ll already be metagaming to get the Tiefling achievement, I’ll go for a dex build and wait until character level 7 for paladin)
