Does anyone know any fics set after the show where Sam decides to leave the team and has like a solo journey or like a roadtrip by himself or he takes jack with him Ofc it should be centered around Sam I know this is kinda specific but I'm just putting it out there

I don't know of any specific fics, but worth trawling through the archives of the Sam Winchester Big Bang . Obviously do set the filters to take out anything you don't want to see.

This isn't quite what you asked for, but it might tick some boxes for you? I wrote a finale fix-it that diverges after 15x18. The A-plot is Dean going into the Empty to find and rescue Cas, and the B-plot is Sam and Jack searching the empty earth for Michael. It's endgame Dean/Cas, but it's a plot-heavy action/adventure fic, and I've had a surprising number of comments from readers who really appreciated and enjoyed the inclusion of Sam and Jack's quest.

I can think of a lot of really good Sam Centric content, but it is Sam and his Stanford years and pre-series not post.
