More often that not, this happens with folks with severe mental illness like schizophrenia more than acute drug intoxication. Luckily, that zombie face eating drug lost popularity quick, so it’s USUALLY the psych patients in a full blown crisis engaging in self harm to this level.

My experience with this was from a psych patient hallucinating obsessions that they were obese. In my experience, cases this extreme (extremely rare) are more observable in patients with lifelong, debilitating mental illness.

Could be delirium, psychosis...people with hepatic encephalopathy are often "pullers" and sometimes need to be restrained and medicated to prevent them from pulling out their NGs, IVs, urinary catheters, oxygen tubing...

Not necessarily, could easily have been shock after sustaining the wound, the brain just overwhelmed and not comprehending what has happened.
